Immunization Requirements and Recommendations

CSU Immunization Requirements and Recommendations

CSU’s Immunization Requirements Policy (EO 803) applies to undergraduate and graduate students, students matriculated in self-support degree programs and K-12 students participating in concurrent enrollment courses entering the California State University (CSU) in or after fall 2023.*

Immunization and Screening, Recommendations and Requirements are adopted from the American College Health Association (ACHA) Immunization Recommendations for College Students and the California Department of Public Health (CDPH) Immunization & Screening Recommendations for College Students.

Any revisions of the CDPH recommendations for colleges and universities as of February 1, each year, will be reflected in CSU recommendations for the subsequent fall academic term.

*Students are defined as all incoming students, with the exception of the following:

  • Students enrolled in fully online degree programs
  • Students enrolled in non-credit, professional development/extended education programs such as Extension or Open University

CSU Required Immunizations

Hepatitis B (Hep B): Ages 18 and younger as per California law (HSC sections 120390-120390-7), enrollees who are 18 years of age or younger are required to provide proof of full immunization against the hepatitis B virus prior to enrollment.

Note: All CSU’s must comply with the baseline requirements of the overarching CSU Immunization Policy, but each university has discretion to expand on these minimum requirements. Please see Cal Poly Humboldt Immunization Policy below for our specific requirements.

CSU Immunizations and Screening Recommendations

The CSU recommends that students are current for the immunizations listed below. Immunizations and screening recommendations, and immunization schedules are linked above. See Cal Poly Humboldt Policy: Required Immunizations  for detailed immunization and screening recommendations. 

Recommended Vaccines to Reduce Outbreaks

  • Influenza Vaccine
  • Measles, Mumps and Rubella (MMR) Vaccine
  • Meningococcal Conjugate (Serogroups A, C, Y, W-135) Vaccine
  • Serogroup B Meningococcal Vaccine
  • Tetanus-Diphtheria-Pertussis (Tdap) Vaccine 
  • Varicella (Chickenpox) Vaccine

Other Vaccines Recommended for Adults

  • Hepatitis A (Hep A) Vaccine
  • Hepatitis B (Hep B) Vaccine - Ages 19 and older
  • Human Papillomavirus (HPV) Vaccine
  • Pneumococcal Vaccine
  • Polio Vaccine

Additional Authorized Discretionary Requirements

A CSU university and/or university-sponsored program whose participants may have increased exposure risk - such as intercollegiate athletics, campus residence halls, or programs involving international travel participants (to or from the United States) - are authorized to require documentation of recommended immunizations in accordance with California Department of Public Health (CDPH) guidelines. Presidents are delegated authority to require vaccination for individuals in consultation with the Chancellor.

In the event of a local or university-based outbreak of a vaccine-preventable disease, the local public health department and/or the CDPH is authorized to require immunizations beyond those currently required by the CSU. Universities will consult with the appropriate public health authority in case of an outbreak.

Academic Program and Fieldwork or Clinical Placement Requirements

All students enrolled in health care fields of study or teacher education programs are required to provide documentation regarding the completion of immunizations and health screenings as established by the state licensing agencies or clinical placement sites or fieldwork sites. Students should contact the academic program for the specific required immunizations, health screenings (e.g., TB screening and physical examination) and any other fieldwork or clinical placement requirements.

Immunization Access

Each university, including self-support degree programs, shall advise students about how to obtain required and recommended immunizations and health screenings through private health care providers, local health departments or the campus. University health centers, in cooperation with local health departments, are authorized to (and may) provide immunizations to enrolled or conditionally enrolled students; however, university health centers are not required to do so.

When applicable, universities, including self-support degree programs, shall advise students about where and how to obtain acceptable proof of full immunization. Examples of acceptable proof of immunization may include a copy of the student’s immunization record, including the name and date(s) of immunization or date and results of blood testing indicating immunity. Acceptable proof of immunization may be provided by the student’s high school, private health care provider, local health department, or campus health center.

Universities have discretion to develop policies and procedures to address students who are not in compliance with any required immunizations.

Each university, including self-support degree programs, are required to develop a consistent process to track compliance with required immunizations and screenings through a designated campus department (e.g., Registrar, Enrollment Management, Student Health Center, Admissions and Records, etc.). The immunization form shall include provisions for medical exemption or religious beliefs. See Cal Poly Humboldt Policy: Required Immunizations below.

CSU Exemptions

A student may be exempted from any requirements in this policy as described below:

Medical Exemption: due to a medical condition for which an Approved Vaccine presents a significant risk of a serious adverse reaction. Any medical Exemption must be verified by a certified or licensed healthcare professional.

Religious Exemption: due to either (i) a person’s sincerely held religious belief, observance, or practice, which includes any traditionally recognized religion, or (ii) beliefs, observances, or practices which an individual sincerely holds and that occupy a place of importance in that individual’s life, comparable to that of traditionally recognized religions.

Cal Poly Humboldt Policy: Immunization Requirements

In accordance with the California State University Policy (EO 803) on immunization requirements, Cal Poly Humboldt (Humboldt) has enacted the following requirements and recommendations.  This policy applies to students enrolling fall 2023 and in subsequent semesters, and is applicable to students who enrolled in the spring 2023 semester.

Cal Poly Humboldt Required Immunizations

Students must be current on the immunizations listed below.  These immunization requirements are a condition of enrollment at Humboldt for students in all undergraduate and graduate students, students matriculated in self-support degree programs, and K-12 students participating in concurrent enrollment courses entering the CSU in or after Fall 2023.  (The policy does not apply to students enrolled in fully online programs or in non-credit, professional development / extended education programs).

For all Students:

Hepatitis B (Hep B): Students age 18 years and younger should complete the series.  Based on the type of vaccine, the series may be two or three doses. (CA Health & Safety Code, Sec. 120390.5)

Measles, Mumps, Rubella (MMR): Two doses with the first dose on or after the first birthday OR a positive titer (laboratory evidence of immunity to disease).

Varicella (chickenpox): Two doses with the first dose on or after the first birthday OR a positive titer (laboratory evidence of immunity to disease).  Note: a personal history of having had chickenpox does not meet compliance.

For Students living in Cal Poly Humboldt Residential Housing:

COVID-19: Individuals must be fully immunized according to the current recommendations of the Centers for Disease Control and Prevention (CDC).

Meningococcal Conjugate (MenACWY): At least one dose on or after age 16 for all students who are ages 21 years or younger. 

Recommended Immunizations

COVID-19: All students, including those living off-campus, including annual boosters in accordance with CDC guidelines.

Hepatitis A (Hep A): All students regardless of age.

Hepatitis B (Hep B): Students that are age 19 and older.

Human papillomavirus (HPV): All students through age 45 years.

Influenza (flu): Annual vaccine for all students regardless of age.

Meningococcal B (Meningitis B): Complete series, may be two or three doses, for students age 16 through 23 years.

Meningococcal Conjugate (MenACWY): for all those entering college between 16 and 23 years of age regardless of housing status (on or off campus); at least one dose at age 16 or older. 

Pneumococcal: For all students age 65 and older and for students with certain medical conditions (e.g., severe asthma, diabetes, chronic liver or kidney disease).

Poliovirus (polio): Regardless of age, if the series was not completed as a child.

Tetanus, diphtheria and pertussis (Tdap): One dose within the last ten years.

Travel immunizations as pertinent based on destination.

Screening / Risk Assessment for Tuberculosis (TB): We recommend that all incoming students complete a risk questionnaire.  Those who are at higher risk for TB infection (as determined by the screening) should undergo testing for TB infection within 30 days of university entry.

Exemptions

Students may submit exemptions to the above immunization requirements based on medical contraindications and/or sincerely held religious beliefs that run counter to all or certain vaccinations.

Enrollment Requirement: Submission of Materials

Students must complete the Student Health and Wellbeing Services (SHWS) Questionnaire about their immunization status (including exemption requests), as well as upload any supporting documentation, through the Cal Poly Humboldt Student Health Portal no later than September 20 for students entering the fall semester, and February 20 for students starting in the spring semester. The form for medical exemptions is available on the SHWS immunization webpage.  SHWS will be responsible for tracking immunization status and communicating issues of non-compliance with the Office of the Registrar.

Holds

Students that are not compliant with Humboldt’s immunization requirements will have their student account put on a “hold.” This hold will apply to the second semester of enrollment and will prevent registration for classes in the second and future semesters. Holds will be removed once a student complies with the immunization requirements and has submitted all necessary materials.

Discretionary Requirements in the Event of an Outbreak

The local public health office and/or the California Department of Health is authorized to require immunizations beyond the requirements above in the event of a local or campus-based outbreak of a vaccine preventable disease. In the event of an outbreak, Humboldt officials will consult with the Humboldt County Department of Health about steps we will take in the interest of public safety.

Academic Program and Fieldwork or Clinical Placement Requirements

Generally speaking, the majority of students will have had most of the required and recommended immunizations during childhood. Where indicated, SHWS will advise students about how to obtain required and recommended immunizations and health screenings through campus, private health care providers, local health departments and pharmacies.
